采用电子计算机平均加算技术,于颅内及颅外记录大脑平均诱发电位并观察针刺对痛觉诱发电位的影响。电刺激一侧手指在对侧头皮或皮层躯体感觉区部位均能记录到多相的复合大脑平均诱发电位,主要由“正—负—正”三个基本波组成。诱发电位的波形组成及波幅与刺激有一定关系,疼痛刺激可见到更为明显的波形。发生感觉障碍时诱发电不明显甚至缺失。观察结果指出针刺能抑制痛觉诱发电位,以对P_2波波幅抑制为最明显(P<0.001)。针刺对皮层诱发电位的抑制作用提示针刺过程促使皮层的兴奋性有所降低,特别是使非特异感觉传导系统的兴奋性有所降低。电针抑制痛觉诱发电位与电针镇痛效果的主观感觉之间可能有较为一致的关系。在电针抑制痛觉诱发电位、镇痛效果的主观感觉与针麻手术评级三者之间似乎也有平行关系。上述结果提示大脑平均诱发电位的某些特点在一定程度上有可能反映疼痛的感觉和针麻过程。
Using computer averaging technique, the average brain evoked potentials were recorded intracranially and extracranially and the effects of acupuncture on pain evoked potentials were observed. The electrical stimulation of one side of the finger in the contralateral scalp or cortex somatosensory area can record the multiphase compound brain average evoked potential, mainly by the “positive - negative - positive” three basic waves. The evoked potential waveform and amplitude and stimulation have a certain relationship, pain stimulation can be seen more obvious waveform. When the sensory disturbance induced electricity is not obvious or even missing. Observations indicate that acupuncture can inhibit the pain-evoked potential to inhibit the amplitude of P 2 wave most significantly (P <0.001). The inhibitory effect of acupuncture on the cortical evoked potentials suggests that the acupuncture process decreases the excitability of the cortex and especially the excitability of the nonspecific sensory transduction system. There may be a more consistent relationship between the electroacupuncture inhibition of pain-evoked potentials and the subjective feeling of electroacupuncture analgesia. There seems to be a parallel relationship between the subjective sense of analgesic effect and acupuncture anesthesia grading in electroacupuncture inhibiting pain-evoked potential. These results suggest that some characteristics of the brain’s average evoked potential may reflect the pain of feeling and acupuncture process to some extent.
