目的对处方用药状况进行调查和点评,充分掌握临床容易出现的不合理处方,提醒药师、医师引起注意,减少不合理用药,提高合理用药水平,确保患者用药安全。方法对照《处方管理办法》、《医院处方点评管理规定管理(试行)》,收集崇州市人民医院2010年2 7月门诊、病区各科室不合理处方,结合药品说明书以及药师查阅的相关文献资料,对其不合理处方进行点评。结果不合理处方包括遴选的药品不适宜、适应证不适宜、用法用量不适宜、联合用药不适宜、重复给药、超常处方。结论合理用药不仅涉及药师、医师,同时涉及护士和患者。只有多方面参与,建立合理用药监督机制,努力开展临床药学监护,才能够提高合理用药水平,减少不合理用药现象的发生。
Objective To investigate and comment on the condition of prescription medication and to fully grasp the unreasonable prescriptions that are prone to appear in clinic so as to remind pharmacists and physicians to pay attention to reduce irrational medication and improve the reasonable medication level so as to ensure the medication safety for patients. Methods According to the “prescription management approach” and “management of hospital prescription review management regulations (for trial implementation)”, we collected the unreasonable prescriptions of Chongzhou People’s Hospital in July 2010 and all departments of ward, combined with the drug specification and the related documents , Comment on its unreasonable prescription. The results of unreasonable prescriptions, including the selection of inappropriate drugs, indications inappropriate, the amount of inappropriate use of combination therapy is not appropriate, repeated administration, prescription. Conclusion Rational medication not only involves pharmacists and physicians, but also nurses and patients. Only by participating in many aspects, establishing a reasonable drug supervision mechanism and working hard to carry out clinical pharmacy monitoring can we raise the level of rational drug use and reduce the occurrence of irrational drug use.
