目的应用fMRI技术,观察左侧颞叶内侧癫痫(MTLE)患者语言区的优势侧别。方法对8名右利手左侧MTLE患者和15名右利手健康志愿者,进行听觉词汇语义判断任务,采集脑功能图像。数据分析采用功能神经成像分析软件(analysis of functional neuroimage,AFNI)进行处理。根据脑功能活动激活结果,计算两侧脑区在语言认知任务上的定侧指数(laterality index,LI)。结果组分析结果显示:MTLE组Broca区和Wernicke区LI平均值分别为-0.29和-0.25,都为右侧半球优势。个体分析显示:MTLE组Broca区6例(75.0%)为不典型语言优势(右侧及双侧);Wernicke区5例(62.5%)为右侧优势。与正常对照组比较,MTLE组Broca区、Wernicke区不典型语言优势例数有显著性差异。结论对左侧MTLE患者而言,语言区的优势半球评价,fMRI是有价值的技术。
Objective To evaluate the superiority of language in the left temporal lobe medial epilepsy (MTLE) by fMRI technique. Methods Eight right-handed left-sided MTLE patients and 15 right-handed healthy volunteers performed auditory vocabularies semantic task to acquire brain function images. Data analysis was performed using the analysis of functional neuroimage (AFNI). According to the activation of brain function activity, laterality index (LI) of bilateral brain regions in language cognitive tasks was calculated. The results of the analysis of the results showed: MTLE group Broca area and Wernicke area LI average -0.29 and -0.25, respectively, for the right hemisphere advantage. Individual analysis showed that 6 cases (75.0%) in Broca area of MTLE group were atypical linguistic advantages (right and bilateral), and 5 cases (62.5%) in Wernicke district were the right superiority. Compared with the normal control group, there were significant differences in the number of atypical language in the Broca area and Wernicke area in the MTLE group. Conclusions For left-sided MTLE patients, fMRI is a valuable technique for assessing the dominant hemisphere in the language region.
