目的:探讨原发性扩张型心肌病(DCM)患者血浆网膜素-1和脂联素水平变化。方法:连续入选100例DCM患者,其中纽约心功能分级Ⅱ级患者22例为DCM1组,Ⅲ级患者45例为DCM2组,Ⅳ级患者33例为DCM3组;另选45例健康者作为对照组。采用酶联免疫吸附法测定网膜素-1和脂联素水平。结果:DCM患者血浆网膜素-1水平显著低于对照组,差异有统计学意义(P<0.01);亚组分析显示,3组DCM患者网膜素-1水平均显著低于对照组,但DCM各亚组间差异无统计学意义。DCM患者血浆脂联素水平显著高于对照组,差异有统计学意义(P<0.01);亚组分析显示,3组DCM患者脂联素水平均显著高于对照组,且DCM3组脂联素水平显著高于DCM1组。DCM患者血浆网膜素-1水平与左室射血分数(LVEF)显著正相关,与脂联素、NT-proBNP、左室舒张末期内径(LVEDD)显著负相关;DCM患者血浆脂联素与CRP、NT-proBNP显著正相关,与网膜素、空腹血糖、LVEF显著负相关。结论:血浆网膜素-1和脂联素水平的变化与DCM发生发展密切相关。
Objective: To investigate the changes of plasma retinulin-1 and adiponectin levels in patients with primary dilated cardiomyopathy (DCM). Methods: A total of 100 patients with DCM were enrolled. Among them, 22 patients with NYHA class Ⅱ were DCM1, 45 patients with DCM were DCM2, 33 patients with IV were DCM3, and 45 healthy controls were selected as control group . The levels of retinulin-1 and adiponectin were determined by enzyme-linked immunosorbent assay. Results: The level of plasma retinin-1 in patients with DCM was significantly lower than that in the control group (P <0.01). Subgroup analysis showed that the levels of retinin-1 in DCM patients were significantly lower than those in the control group However, there was no significant difference between DCM subgroups. The levels of plasma adiponectin in patients with DCM were significantly higher than those in the control group (P <0.01). The subgroup analysis showed that the levels of adiponectin in DCM patients were significantly higher than those in the control group The level was significantly higher than DCM1 group. The level of plasma retinin-1 in DCM patients was positively correlated with left ventricular ejection fraction (LVEF) and negatively correlated with adiponectin, NT-proBNP and left ventricular end-diastolic diameter (LVEDD). Plasma adiponectin CRP, NT-proBNP was significantly and positively correlated with retinal, fasting blood glucose, LVEF significant negative correlation. Conclusion: The changes of plasma retinulin - 1 and adiponectin levels are closely related to the occurrence and development of DCM.
