目的研究不同血压水平对2型糖尿病人群肾功能的影响。方法采用前瞻性队列研究方法,共收集5 463例体检糖尿病患者,以基线不同血压水平分为5组,以eGFR<60mL/(min·1.73m2)定义为肾功能不全作为终点事件,应用多因素Logistic回归模型分析血压及其他因素对肾功能的影响。结果 (1)观察对象中新发肾功能不全者为454例,5组患者发病率分别为5.5%、6.6%、8.9%、9.4%和14.7%(P<0.01)。(2)多因素Logistic回归分析显示,与第1组比较,第2组到第5组发生肾功能不全事件的RR值分别为:1.12(95%CI:0.79~1.59,P=0.52)、1.44(95%CI:1.02~2.03,P=0.04)、1.40(95%CI:1.02~1.93,P=0.04)、2.17(95%CI:1.51~3.12,P<0.01)。结论高血压可增加糖尿病人群肾功能不全发病风险。
Objective To study the effects of different blood pressure levels on renal function in type 2 diabetic patients. Methods A prospective cohort study was conducted. A total of 5 463 patients with physical diabetes were collected and divided into 5 groups according to baseline blood pressure. EGFR <60 mL / (min · 1.73 m2) was defined as renal insufficiency as the end point. Multiple factors Logistic regression model was used to analyze the effect of blood pressure and other factors on renal function. Results (1) Among the 454 newly diagnosed renal insufficiency patients in the observation group, the incidence rates of the five groups were 5.5%, 6.6%, 8.9%, 9.4% and 14.7%, respectively (P <0.01). (2) Multivariate logistic regression analysis showed that the RRs of renal insufficiency in group 2 to group 5 were 1.12 (95% CI: 0.79-1.59, P = 0.52), 1.44 (95% CI: 1.02-2.03, P = 0.04), 1.40 (95% CI: 1.02-1.93, P = 0.04), 2.17 (95% CI: 1.51-3.12, P <0.01). Conclusion Hypertension can increase the risk of renal insufficiency in diabetic population.
