目的探讨不同剂量纳洛酮救治急性有机磷农药中毒(AOPP)早期伴呼吸衰竭的临床疗效。方法本组41例随机分为2组,纳洛酮0.8 mg 组与0.4 mg 组。在传统治疗的基础上,两组应用不同剂量纳洛酮,同时要求及时使用机械通气。结果二组在临床显效率、有效率统计学上均有显著差异(P<0.05)。0.8 mg 组死亡2例,分别死于多脏器功能衰竭和急性肾功能衰竭;0.4 mg 组死亡3例,其中2例死于急性呼吸衰竭,1例死于多脏器功能衰竭。结论在救治 AOPP 早期伴呼吸衰竭的危重患者时,较大剂量纳洛酮方案(0.8 mg 组)比小剂量纳洛酮(0.4 mg 组)更有效。
Objective To investigate the clinical effects of different doses of naloxone in the treatment of acute organophosphorus pesticide poisoning (AOPP) early with respiratory failure. Methods The group of 41 patients were randomly divided into two groups, naloxone 0.8 mg group and 0.4 mg group. On the basis of traditional treatment, two groups of different doses of naloxone, while requiring the timely use of mechanical ventilation. The results of the two groups in the clinical significant efficiency, effective statistics were significantly different (P <0.05). 0.8 mg group died in 2 cases, died of multiple organ failure and acute renal failure respectively; 3 patients died in 0.4 mg group, 2 of them died of acute respiratory failure and 1 died of multiple organ failure. Conclusions Naloxone at a higher dose (0.8 mg) is more effective than naloxone at a lower dose (0.4 mg) in the treatment of critically ill patients with early AOPP and respiratory failure.
