目的探讨新疆维吾尔族男性及女性瘦素水平与代谢综合征(MS)的关系及差异。方法从新疆和田地区维吾尔族流行病学调查的数据库中,随机选取781例个体,年龄30~70岁。其中,MS组403例、对照组378例,男性249例、女性532例。比较男性及女性在不同瘦素水平时MS的发生频率及其各组分的差异;并采用多元线性回归的方法,分析男性及女性瘦素水平的主要影响因素。结果新疆维吾尔族男性瘦素的中位数为0.58ng/ml,女性瘦素的中位数为1.76ng/ml,女性的瘦素水平约为男性的3倍(P<0.01)。男性及女性中MS组的瘦素水平均明显高于对照组(P<0.01);随着瘦素水平的递增,男性及女性的MS分布频率、腰围(WC)、体重指数(BMI)、舒张压(DBP)、甘油三酯(TG)、胰岛素抵抗指数(HOMA-IR)均逐步升高(P<0.05)。多元线性回归分析显示,男性的瘦素水平与Ln HOMA-IR(B=0.179,P<0.05)及BMI(B=0.077,P<0.05)呈正相关;女性的血清瘦素水平与BMI呈正相关(B=0.067,P<0.001),与年龄呈负相关(B=-0.012,P<0.01)结论新疆维吾尔族男性及女性中MS患者的瘦素水平均明显升高,瘦素水平与MS的发生频率及BMI均呈正相关;但存在性别差异,男性的瘦素水平与胰岛素抵抗呈正相关,而女性的瘦素水平与年龄呈负相关。
Objective To investigate the relationship between leptin levels and metabolic syndrome (MS) in Uighur men and women in Xinjiang Uygur Autonomous Region. Methods A total of 781 individuals were selected from the Uyghur epidemiological survey database in Hotan, Xinjiang, aged from 30 to 70 years. Among them, 403 cases in MS group and 378 cases in control group were 249 males and 532 females. To compare the frequency of MS in different levels of leptin between males and females and the differences of their components. The main influencing factors of leptin level in males and females were analyzed by multivariate linear regression. Results The median number of male leptin was 0.58ng / ml in Uygur nationality in Xinjiang. The median leptin level in female was 1.76ng / ml. The leptin level in female was three times of that in male (P <0.01). The levels of leptin in MS group were significantly higher than those in control group (P <0.01). The distribution of MS, waist circumference (WC), body mass index (BMI) The levels of DBP, TG and HOMA-IR increased gradually (P <0.05). Multivariate linear regression analysis showed that there was a positive correlation between leptin levels and Ln HOMA-IR (B = 0.179, P <0.05) and BMI (B = 0.077, P <0.05) B = 0.067, P <0.001). There was a negative correlation with age (B = -0.012, P <0.01) Conclusion The leptin levels of MS patients in Uygur nationality in Xinjiang and Uygur were significantly increased, Frequency and BMI. However, there was a gender difference. There was a positive correlation between leptin level and insulin resistance in males, while the leptin level in females was negatively correlated with age.
