女性,42岁。乏力、盗汗、午后低热半年,伴膝、踝关节游走性疼痛,严重时行走困难。胫前皮肤和膝关节周围疼痛,无咳嗽、喀痰、胸背胀痛。体查:体温38℃、皮肤、球结膜无黄染,全身浅表淋巴结不肿大。双肺呼吸音清晰。心率90次,律齐,未闻杂音。腹软无压痛,肝脾肋下未及,肠鸣音正常。膝、踝关节红肿,膝关节、胫前皮肤见数个结节性红斑、压痛明显。肝功能正常,抗核抗体(一),类风湿因子(一),抗“O”<200u,血沉20mm/h,OT试验(1:1万)++。
