目的:研究急性坏死性胰腺炎的CT影像学分级与预后的关系。方法:39例AHNP病人入院后72h内行动态CT检查,按CT影像学的表现进行分级,总分≤3分为轻型,4~5分为中型,≥6分为重型。将按CT影像学分级的不同类型病人的病情严重程度及预后判别指标进行统计分析。结果:39例中,轻型12例,中型17例,重型10例。轻型病人的临床恢复过程较重型顺利,轻型及中型病人中转手术率及死亡率均较重型病人明显为低。结论:本CT影像学分级可以用于急性坏死性胰腺炎非手术治疗病人的病情判别及预后评估。
Objective: To study the relationship between CT grade and prognosis of acute necrotizing pancreatitis. Methods: Thirty-nine patients with AHNP underwent dynamic CT within 72 hours after admission. The patients were classified according to the CT findings. The total score ≤3 was classified as light, 4 to 5 as medium, and ≥6 as heavy. Will be classified according to CT imaging of different types of patients the severity of the disease and prognostic indicators for statistical analysis. Results: Among 39 cases, 12 were mild, 17 were medium and 10 were heavy. The clinical recovery of patients with light-weight was smoother and heavier. The rates of trans-operation and mortality in both light and medium-sized patients were significantly lower than those in the heavier patients. Conclusion: This CT imaging grade can be used for the diagnosis and prognosis of patients with non-surgical treatment of acute necrotizing pancreatitis.
