为探索原发性高血压肺血流图变化,本组观察100例高血压病人并以100名正常人作对照。观察结果表明: (1)波型:高血压组单峰型者居多, 而正常对照组多为正常波型。(2)时间:在反映右心功能及肺动脉的各项指标(Q-j间期,j—Z间期及其Q—j/j-Z 值)均无明显变化。(3)波幅: 正常组hc/hz值0.56, 高血压组hc/hz值0.68,因此,高血压组hc/hz值明显高于对照组。在本组观察的高血压病人中,多数可见到因肺静脉系统容量增多所形成的α波,次波(c)相对增大,主波(z)减低,由此hc/hz值增大的肺血图表现,在本组所观察的100例中hc/hz>0.66者占20%,明显高于对照组,可能提示高血压病均有不同程度左心前负荷增加倾向。按左心前负荷分级评定,本组在Ⅱ级以上占20%。因此,高血压病左心前负荷增加多在轻度至中度范围,常无临床症状,肺血流图为高血压病左心前负荷的程度提供了一个客观指标。基于高血压病可有左心前负荷增加的肺血图表现,适当给予降低前负荷治疗对防止其病变发展可能有益。
In order to explore the changes of pulmonary blood flow in patients with essential hypertension, we observed 100 hypertensive patients and compared them with 100 healthy controls. The results showed that: (1) wave type: hypertension group unimodal majority, while the normal control group mostly normal wave type. (2) Time: There was no significant change in the indexes of right heart function and pulmonary artery (Q-j interval, j-Z interval and Q-j / j-Z value). (3) The amplitude: the hc / hz value of normal group 0.56, hypertension group hc / hz value of 0.68, therefore, hypertension group hc / hz value was significantly higher than the control group. In this group of patients with hypertension observed, the majority can be seen due to increased pulmonary venous system formed by the α wave, the relative increase of the secondary wave (c), the main wave (z) decreased, thus hc / hz increased lung Blood charts, observed in this group of 100 cases hc / hz> 0.66 accounted for 20%, was significantly higher than the control group, may prompt hypertension have varying degrees of left ventricular preload increase. According to the left heart load rating, the group accounted for 20% above grade Ⅱ. Therefore, the increase of left ventricular preload in hypertensive patients is mostly in the mild to moderate range, often without clinical symptoms. The pulmonary blood flow diagram provides an objective index for the extent of left ventricular preload in hypertensive patients. Hypertension may have increased left ventricular load in the lung blood diagram showed that appropriate to reduce pre-load treatment to prevent the development of its disease may be beneficial.
