目的观察肿瘤相关糖蛋白-72(TAG-72)抗原在结肠癌及癌前病变组织中的表达,并探讨其临床意义。方法应用免疫组化EnVision二步法检测TAG-72抗原在221例结肠癌及癌前病变组织中的表达,其中癌旁正常结肠黏膜32例、结肠增生性息肉24例、结肠低级别上皮内瘤变75例、结肠高级别上皮内瘤变和结肠管状腺癌各45例。结果 TAG-72抗原在正常结肠黏膜、增生性息肉、低级别上皮内瘤变、高级别上皮内瘤变及腺癌组的表达阳性率分别为3.12%、29.17%、53.33%、88.89%、100%。正常组与息肉组间表达率差异不显著(P>0.05);低级别、高级别上皮内瘤变组、腺癌组抗原表达率显著高于前两组(P<0.05);其中高级别上皮内瘤变组与腺癌组的抗原表达明显增高,但两组间表达差异不显著(P>0.05)。结论 TAG-72在结肠高级别上皮内瘤变及结肠癌中表达增高。由于TAG-72在结肠组织的恶性转化中起着重要作用,故其阳性表达及表达强度对诊断结肠腺癌和监测腺瘤癌变有重要意义。
Objective To observe the expression of tumor-associated glycoprotein-72 (TAG-72) antigen in colon cancer and precancerous lesions and to investigate its clinical significance. Methods The immunohistochemical EnVision two-step method was used to detect the expression of TAG-72 antigen in 221 cases of colon cancer and precancerous lesions. Among them, there were 32 cases of normal colonic mucosa, 24 cases of colon hyperplastic polyps, Change in 75 cases, high-grade colon intraepithelial neoplasia and colon tubular adenocarcinoma in 45 cases. Results The positive rates of TAG-72 antigen in normal colonic mucosa, proliferative polyps, low grade intraepithelial neoplasia, high grade intraepithelial neoplasia and adenocarcinoma were 3.12%, 29.17%, 53.33%, 88.89%, 100 %. There was no significant difference in the expression rates between the normal group and the polyp group (P> 0.05). The expression rates of antigen in low grade, high grade intraepithelial neoplasia group and adenocarcinoma group were significantly higher than those in the first two groups (P <0.05) The expression of antigen in the group of neoplasia and adenocarcinoma was significantly increased, but there was no significant difference between the two groups (P> 0.05). Conclusion TAG-72 is highly expressed in high-grade colorectal neoplasia and colon cancer. Since TAG-72 plays an important role in the malignant transformation of colon tissue, its positive expression and expression intensity are of great significance for the diagnosis of colon adenocarcinoma and the monitoring of adenoma carcinogenesis.
