目的:通过分析前列腺外周带癌灶与炎症的磁共振成像(MRI)特点,探讨多参数磁共振成像(Mp-MRI)对前列腺外周带癌灶与炎症的临床鉴别价值。方法:回顾性分析2017年1月至2019年6月本院收治的78例前列腺外周带可疑癌灶患者的临床资料,所有患者均通过Mp-MRI检查后行经会阴前列腺穿刺术,术后病理证实为前列腺癌患者38例,前列腺急慢性炎症者40例。统计分析所有患者的T2加权成像(T2WI)、Mp-MRI如扩散成像技术(DWI)、动态增强扫描技术(DCE-MRI)、表观扩散系数图(ADC)值的特征。结果:前列腺外周带癌灶低信号边缘模糊的频率、T2WI弥漫性低信号及T2WI不均匀低信号均低于前列腺外周带炎症(n P<0.05);高b值DWI上前列腺外周带癌灶的稍高信号出现频率低于前列腺外周带炎症(n P<0.05);前列腺癌患者信号强度-时间(SI-T)的分布明显高于前列腺外周带炎症,差异均有统计学意义(n P<0.05);前列腺外周带癌灶的ADC值明显低于前列腺外周带炎症,ROC曲线下面积为0.881,最佳临界点为0.892×10n -3 mmn 2/s,前列腺外周带炎症诊断的特异度为70.2%,敏感度为93.2%。n 结论:前列腺Mp-MRI的DWI稍高信号强度、DCE的SI-T分型和ADC值有助于鉴别前列腺外周带病灶的良恶性。“,”Objective:To analyze the MRI characteristics of prostate peripheral cancer and inflammation, to discuss the clinical value of Mp-MRI in differentiating prostate peripheral cancer and inflammation.Methods:Retrospective analysis of clinical data of 78 patients with suspicious peripheral prostate cancer foci in our hospital in January 2017 to June 2019 was performed, all patients underwent MRI of Mp-after menstruation perineal prostate puncture, postoperative pathology confirmed 38 cases of prostate cancer and 40 cases of acute or chronic inflammation of prostate.Characteristics of all patients of T2WI, DWI, DCE, ADC values were analyzed.Results:The frequency of low signal edge blurring, diffuse low signal T2WI and uneven low signal T2WI in peripheral prostate cancer lesions were all lower than those in peripheral prostate cancer lesions(n P<0.05). On DWI with high b value, the frequency of slightly higher signal in peripheral prostate cancer lesions was lower than that in peripheral prostate inflammation lesions(n P<0.05). The differences in signal intensive-time (SI-T) between the two groups were statistically significant(n P<0.05). The ADC value of peripheral prostate cancer lesions was significantly lower than that of peripheral prostate inflammation lesions. The area under ROC curve was 0.881, and the optimal critical point was 0.892×10n -3mmn 2/s.The specificity of diagnosis of peripheral prostate cancer lesions was 70.2%, and the sensitivity was 93.2%.n Conclusions:The slightly higher DWI signal intensity of prostate Mp-MRI, SI-T classification of DCE and ADC value are helpful for the identification of benign and malignant peripheral lesions of prostate.
