目的:动态地了解慢性闭角型青光眼(慢闭青)行YAG激光虹膜周切术(YLI)前、后的前房角定量改变,和虹膜膨隆的形态改变。方法:随机对确诊为慢闭青的门诊患者11人18眼,经房角镜检查按Scheie分类法均为窄2~窄3、4之间,裂隙灯检查前房下角均<1/3CT。眼压>21mmHg,C/D0.2~0.5,接受药物治疗的患者。在行YLI之前和YLI之后,用ACUSON128XP/10型电脑声像仪各行一次超声房角测量,同时观察虹膜膨隆形态的改变。结果:超声测量显示手术前、后,前房角的平均宽度有明显改变,YLI前为15.22°±5.87°,YLI后为24.44°±5.96°。YLI后周边虹膜不再向前膨隆,前房下角>1/3CT。结论:ACUSON12XP/10型电脑声像仪可用以定量测量活体前房角宽度,超声测量显示慢闭青施行激光虹膜周切术后,前房角明显增宽,虹膜膨隆变平。
Objective: To dynamically understand the quantitative changes of anterior chamber angle and the morphological changes of iris bulging before and after YAG laser iridectomy (YLI) in patients with chronic angle-closure glaucoma (slow-closed glaucoma). Methods: A total of 11 outpatients with 18 eyes were diagnosed as slow-closed patients. According to Scheie’s classification, they were all narrow 2 ~ narrow 3 and 4 by glottic examination. The anterior chamber angle was <1 / 3CT for slit lamp examination. IOP> 21mmHg, C / D0.2 ~ 0.5, patients receiving drug therapy. Prior to YLI and YLI, an ultrasound scan was performed with an ACUSON 128XP / 10 computer imager, and the change in iris bulging was observed. Results: Ultrasound measurements showed that the average width of anterior chamber angle varied significantly from 15.22 ° ± 5.87 ° before YLI to 24.44 ° ± 5.96 ° after YLI. YLI peripheral iris is no longer bulging forward, anterior chamber angle> 1 / 3CT. Conclusion: The ACUSON 12XP / 10 computer imager can be used to quantitatively measure the anterior chamber width of the living body. Ultrasound measurements show that the angle of the anterior chamber is obviously broadened and the iris bulges and flattens.
