目的探讨腹腔镜下子宫肌瘤剔除术对患者体内免疫功能及氧化应激水平的影响。方法选取2015年6月-2016年10月该院132例子宫肌瘤患者,65例行腹腔镜子宫肌瘤剔除术的为腹腔镜组,67例行开腹子宫肌瘤剔除术的子宫肌瘤患者为开腹组。分别于术前,术后24 h、72 h取两组患者的血浆,对血浆中氧化应激相关因子,如活性氧(ROS)、丙二醛(MDA)、晚期氧化蛋白产物(AOPP)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GPx),总抗氧化能力(AOA)水平,血浆中免疫相关参数,如白细胞计数(WBC)、中性粒细胞比率(NGR)、Th1/Th2细胞比值、C反应蛋白(CRP)及白细胞介素水平。结果术后24 h、72 h两组患者血浆中ROS、MDA、AOPP、CRP、IL-1及IL-6水平相比于术前显著升高(P<0.05),而腹腔镜组患者血浆中ROS、MDA、AOPP、CRP、IL-1及IL-6水平显著低于开腹组(P<0.05);两组患者术后24 h、72 h血浆中SOD、GPx、AOA相比于手术前显著下降(P<0.05),而腹腔镜组患者血浆中SOD、GPx及AOA水平显著高于开腹组(P<0.05);与术前相比,术后24 h、72 h两组患者血浆中WBC、NGR显著上升(P<0.05),而Th1/Th2细胞比值显著下降(P<0.05),腹腔镜组患者血浆中WBC、NGR显著低于开腹组(P<0.05),Th1/Th2细胞比值显著高于开腹组(P<0.05)。结论腹腔镜下子宫肌瘤剔除术对患者体内免疫功能及氧化应激水平的影响显著弱于开腹手术。
Objective To investigate the effect of laparoscopic myomectomy on immune function and oxidative stress in patients. Methods From June 2015 to October 2016, 132 patients with uterine fibroids in our hospital were enrolled. Among them, 65 patients underwent laparoscopic myomectomy for laparoscopic group and 67 patients for uterine fibroids on laparoscopic myomectomy The patient was an open group. Plasma concentrations of oxidative stress-related factors, such as reactive oxygen species (ROS), malondialdehyde (MDA), late oxidized protein product (AOPP) The levels of superoxide dismutase (SOD), glutathione peroxidase (GPx), total antioxidant capacity (AOA), plasma immune parameters such as WBC, neutrophil ratio ), Th1 / Th2 ratio, C-reactive protein (CRP) and interleukin levels. Results The plasma levels of ROS, MDA, AOPP, CRP, IL-1 and IL-6 in the two groups at 24 h and 72 h after operation were significantly higher than those before operation (P <0.05) The levels of ROS, MDA, AOPP, CRP, IL-1 and IL-6 in the two groups were significantly lower than those in the open group (P <0.05) (P <0.05), while the levels of SOD, GPx and AOA in the laparoscopic group were significantly higher than those in the laparotomy group (P <0.05). Compared with the preoperative level, the plasma levels of plasma (P <0.05), while the ratio of Th1 / Th2 cells decreased significantly (P <0.05). The plasma WBC and NGR in laparoscopic group were significantly lower than those in open group (P <0.05), Th1 / Th2 Cell ratio was significantly higher than the open group (P <0.05). Conclusions The effect of laparoscopic myomectomy on immune function and oxidative stress level in patients is significantly weaker than that of laparotomy.
