患者女,37岁.因胆总管下段结石并梗阻.拟行连续硬膜外阻滞胆道探查术.既往无药物过敏史.普鲁卡因皮试阴性,术前肌注苯巴比妥纳0.1g,阿托品0.5mg;血压、脉搏、呼吸正常.取右侧卧位,常规消毒皮肤铺巾后穿刺T_(8-9)椎间隙置管4cm(向头端),即注入2%利多卡因(江苏泰兴县制药厂,批号92307一1)4mL观察效果.4min后患者诉腹部有发热感,用针试皮时答话声嘶,出现呼吸困难,脉细弱,口唇发绀,颈胸、管遍布红色斑疹.即改平卧面罩给氧,但血压测不出,心跳、呼吸停止.立即施行胸外按压,紧急气管插管手控呼吸,静注肾上腺素lmg,快速静滴5%.碳酸氢钠250mL、地塞米松30mg.约lmin后心跳,
