Background: Health departments in Khartoum, the Red Sea, Gadarifu, Kassala, Jebel, Sudila and Kitakordur in Sudan. Purpose: To study the relationship between symptoms and clinical history of tuberculosis patients and the positiveness of smear. Methods: 5338 suspicious TB patients aged from 15 to 49 years of age, March 1998 to March 1999 treatment. They were both sputum smear microscopy and had sputum smear results according to standard methods. With personal consent, patients are given a structured initial visit to their symptoms and their duration and presence of a family affiliation. Statistical analysis determined the relationship between symptoms and family characteristics and smear test results. RESULTS: A total of 514 suspicious individuals (9.6%) were detected for acid-fast bacilli: 45 (8.8%) at +1, 167 (32.5%) at +2 and 302 (58.8%) at +3, Most patients have symptoms for 6-9 weeks, while 100 have been older than a year, many of whom (47) have other chronic respiratory diseases. The majority of chest symptoms were related to the positive sputum smear but not to the duration of symptoms (P = 0.018, P = 0.028, respectively). Those with symptoms lasting less than one year had a high positive sputum smear (+3) and the longest duration of symptoms, a clear hallmark of disease at home. (OR = 1.99, 95% CI 1.12-3.55, OR = 1.004, 95% CI 1.000-1.008). CONCLUSIONS: This study shows that there is a relationship between the positive sputum smear level and the clinical features of TB patients.
