目的探讨胃癌组织及血清中肝肠-钙黏连蛋白(CDH17)、糖类抗原24-2(CA24-2)、糖类抗原72-4(CA72-4)与癌胚抗原(CEA)的表达水平及临床意义。方法选取石河子大学医学院第一附属医院40例胃癌患者、40例胃良性病变患者及40例健康体检者,采用酶联免疫吸附测定法(ELISA)和电化学发光法检CDH17、CA24-2、CA72-4、CEA 4项指标在血清中的浓度,采用免疫组化方法观察CDH17、CA24-2、CA72-4、CEA在胃黏膜组织中的表达情况。结果胃癌患者血清CDH17、CA24-2、CA72-4、CEA含量明显高于胃良性病变组及健康对照组,差异有统计学意义(P<0.05);CDH17、CA24-2、CA72-4、CEA在胃癌组织中的阳性表达率明显高于胃良性病变组及健康对照组,差异有统计学意义(P<0.05);CDH17、CA24-2、CA72-4、CEA在胃癌患者血清中的含量可以反映其在肿瘤组织中的表达,二者水平呈正相关;联合检测CDH17、CA24-2、CA72-4、CEA在胃癌中的灵敏度和特异度分别为87.5%和81.25%,与单项检测胃癌灵敏度和特异度相比较,差异有统计学意义(P<0.05)。结论联合检测肿瘤标志物CDH17、CA24-2、CA72-4、CEA可提高胃癌诊断的灵敏度,有利于胃癌的早期诊断和治疗。
Objective To investigate the expression of CDH17, CA24-2, CA72-4 and CEA in gastric cancer tissue and serum. And clinical significance. Methods Forty patients with gastric cancer, 40 patients with benign gastric lesions and 40 healthy subjects were selected from the First Affiliated Hospital of Shihezi University School of Medicine. Enzyme-linked immunosorbent assay (ELISA) and electrochemiluminescence were used to examine CDH17, CA24-2, CA72-4 and CEA were measured in serum. Immunohistochemistry was used to observe the expression of CDH17, CA24-2, CA72-4 and CEA in gastric mucosa. Results Serum levels of CDH17, CA24-2, CA72-4 and CEA in patients with gastric cancer were significantly higher than those in patients with benign gastric lesions and healthy controls (P <0.05); CDH17, CA24-2, CA72-4, CEA The positive rates of CDH17, CA24-2, CA72-4 and CEA in gastric cancer tissues were significantly higher than those in benign gastric lesions and healthy controls (P <0.05) The sensitivity and specificity of combined detection of CDH17, CA24-2, CA72-4 and CEA in gastric cancer were 87.5% and 81.25% respectively, which were significantly different from those of single detection of gastric cancer sensitivity and Specificity, the difference was statistically significant (P <0.05). Conclusion Combined detection of tumor markers CDH17, CA24-2, CA72-4, CEA can improve the sensitivity of gastric cancer diagnosis, which is good for the early diagnosis and treatment of gastric cancer.
