前列环素(PGI_2)及它的同类物具有强烈的扩血管特性,在人体内可使体、肺循环及冠脉阻力明显降低。然而阻力血管扩张可能对重度冠脉狭窄和冠脉储备已耗竭患者的冠脉血流动力学有不良作用。冠脉压进一步下降和/或持续的心动过速又易引起心肌缺血。本文旨在估价PGI_2和它的稳定同类物Iloprost引起心肌缺血的机制,并为避免该药的副作用提出预防性措施。
Prostacyclin (PGI_2) and its congeners have a strong vasodilator properties that significantly reduce body, pulmonary circulation and coronary resistance in humans. However, resistance to vasodilation may have adverse effects on coronary hemodynamics in patients with severe coronary artery stenosis and depleted coronary artery disease. A further decline in coronary pressure and / or sustained tachycardia may also cause myocardial ischemia. This article aims to evaluate the mechanism by which PGI_2 and its stable congener Iloprost cause myocardial ischemia and to propose prophylactic measures to avoid side effects of this drug.
