目的:对糖尿病患者尿糖水平对尿液白细胞干化学法测定的影响及尿液中炎性细胞因子的变化进行分析研究,确定尿糖对尿白细胞干化学的影响与程度及其与尿液炎性细胞因子变化的相关性。方法:收集2014-12-2015-04尿液常规标本,用显微镜镜检筛选出尿液白细胞阳性的病例,根据尿糖结果分为尿糖阴性组(-)、阳性组(微量组±、1+组、2+组、3+组和4+组),分析这6组标本尿液干化学白细胞检测结果的差异,设计实验进行验证,同时检测各组的炎性细胞因子的变化,作相关性分析。结果:尿糖阴性组与阳性组的白细胞干化学检查结果存在显著差异(P<0.05),随着尿糖浓度升高到2+(28 mmol/L)及以上时,差异越显著(P<0.05);且随着尿糖浓度升高,干化学法白细胞检测的假阴性率越高(P<0.05),此时炎性细胞因子亦增高(P<0.05)。结论:尿糖浓度升高(28mmol/L及以上时)会对尿液白细胞干化学法检查结果产生明显影响,在临床检验工作中,对尿糖阳性(特别是尿糖28mmol/L及以上)的样本应及时显微镜复检,以确保尿液白细胞检测结果的准确性。
OBJECTIVE: To analyze the effect of urine glucose level on the determination of leucocyte dry-chemistry in urine and the changes of inflammatory cytokines in urine to determine the influence and degree of urinary sugar on leucocyte dry-chemistry and its relationship with urinary inflammation Correlation of changes in sex cytokines. Methods: Urine routine samples were collected from December 2014 to April 2015. Urine leucocyte positive cases were selected by microscopic examination. Results: There was a significant difference (P <0.05) in the results of leukocyte chemotaxis between urine negative group and positive group, and the more significant difference was when urine glucose concentration increased to 2+ (28 mmol / L) and above (P < 0.05). With the increase of urinary glucose concentration, the false-negative rate of dry chemical leukocyte test was higher (P <0.05), and inflammatory cytokines were also increased at this time (P <0.05). CONCLUSION: Urinary glucose concentration (28 mmol / L and above) can significantly affect urine leucocyte chemosensitivity test results. Positive urine sugar (especially urine sugar 28 mmol / L and above) The samples should be timely microscopic examination to ensure the accuracy of urine leukocyte test results.
