目的探讨脑白质疏松症(LA)患者认知功能水平与脑血流动力学的关系。方法对90例LA患者进行颅脑MRI、神经心理学与TCD检查,根据评分将其分为3组:无认知功能障碍组、轻度认知功能障碍组和痴呆组。运用方差分析及相关分析来研究LA患者不同认知功能水平与大脑中动脉(MCA)搏动指数(PI)、平均血流速度(Vm)的关系。结果 PI在无认知功能障碍组、轻度认知功能障碍组和痴呆组之间差异具有统计学意义(P<0.05),且PI与LA患者的认知功能水平为正相关关系(r=0.688,P<0.01)。Vm在三组患者之间差异无统计学意义(P>0.05)。结论 LA患者认知功能水平与大脑中动脉PI有关,PI值越大,LA患者认知功能障碍程度越重。LA患者的认知功能水平与大脑中动脉Vm无明显相关关系。
Objective To investigate the relationship between cognitive function and cerebral hemodynamics in patients with leukoaraiosis (LA). Methods Ninety LA patients underwent brain MRI, neuropsychology and TCD examination. The patients were divided into three groups according to their scores: no cognitive impairment group, mild cognitive impairment group and dementia group. ANOVA and correlation analysis were used to investigate the relationship between different cognitive functions and middle cerebral artery (MCA) pulsatility index (PI) and mean blood flow velocity (Vm) in LA patients. Results There was a significant difference in PI between cognitive impairment group, mild cognitive impairment group and dementia group (P <0.05), and there was a positive correlation between PI and cognitive function in LA patients (r = 0.688, P <0.01). There was no significant difference in Vm between the three groups (P> 0.05). Conclusions The cognitive function of LA patients is related to the middle cerebral artery (PI). The larger the PI value, the more severe the cognitive dysfunction in LA patients. There was no significant correlation between cognitive function and Vm of middle cerebral artery in LA patients.
