目的分析三阶梯镇痛治疗对介入科收治的恶性肿瘤伴疼痛者的临床应用及安全性。方法恶性肿瘤伴疼痛者80例,根据疼痛程度,分别给以不同阶梯的镇痛治疗并观察临床疗效及不良反应,用数字评分法评估疼痛程度及生活质量。根据肿瘤情况,施行不同的介入治疗。结果治疗前疼痛程度评分均值为6.86,治疗后第3,7,14天疼痛程度评分分别降至3.12,2.40,2.01,疼痛程度显著减轻(P<0.01),总有效率为98.75%。治疗后生活质量明显改善(P<0.01)。不良反应总发生率约41.25%,多为轻度.主要为便秘、恶心、呕吐、头晕、嗜睡,未见呼吸抑制等严重不良反应。结论镇痛治疗对介入科癌痛患者重要实用,应给以积极的关注及治疗,随时评估镇痛疗效,调整用药剂量,纠正不良反应。
Objective To analyze the clinical application and safety of three-step analgesic for the treatment of malignant tumors with pain in the interventional department. Methods Eighty patients with malignant tumor and pain were divided into two groups. According to the degree of pain, different levels of analgesic therapy were given respectively. The clinical efficacy and adverse reactions were observed. The pain scores and quality of life were evaluated by digital scoring method. Depending on the condition of the tumor, different interventions are performed. Results The mean score of pain before treatment was 6.86. The pain scores on the 3rd, 7th and 14th days after treatment decreased to 3.12, 2.40 and 2.01 respectively. The pain was significantly relieved (P <0.01). The total effective rate was 98.75%. After treatment, the quality of life was significantly improved (P <0.01). The total incidence of adverse reactions was about 41.25%, mostly mild, mainly constipation, nausea, vomiting, dizziness, drowsiness, no respiratory depression and other serious adverse reactions. Conclusion The analgesic treatment is important and practical for interventional cancer patients with pain. Positive attention and treatment should be given to evaluate the analgesic efficacy, adjust dosage and correct adverse reactions.
