目的:探讨产前诊断单脐动脉(single umbilical artery,SUA)及其合并心血管畸形的意义。方法:回顾分析SUA胎儿230例,分析其合并心脏畸形概率及畸形构成比,并对其中143例SUA胎儿脐动脉的缺失侧和心脏畸形的关系作分析。结果:本中心产前胎儿超声心动图检出胎儿SUA检出率为2.2%,合并胎儿心脏畸形71例(30.9%,71/230);143例SUA中,左侧脐动脉缺失81例(56.5%),右侧62例(43.4%),单脐带动脉胎儿右侧较左侧脐动脉缺失更易合并心脏畸形(P=0.035)。结论:胎儿单脐动脉合并心脏畸形的概率增加,SUA合并心脏畸形以圆锥动脉干畸形及室间隔缺损最为常见。SUA胎儿左侧脐带缺失比例大于右侧,右侧脐动脉缺失的胎儿更易合并心脏畸形。
Objective: To investigate the significance of prenatal diagnosis of single umbilical artery (SUA) and its associated cardiovascular malformations. Methods: 230 SUA fetuses were retrospectively analyzed. The probability of malformation and the ratio of malformations were analyzed. The relationship between missing side and heart malformation of 143 fetuses with SUA fetuses was analyzed. Results: Prenatal fetal echocardiography was used to detect fetal SUA in 2.2% of cases and 71 cases of fetal malformation (30.9%, 71/230). Among 143 cases of SUA, 81 cases (56.5%) were missing in left umbilical artery %), The right side of the 62 cases (43.4%), single umbilical cord fetal right lower left umbilical artery is more likely to be associated with cardiac malformations (P = 0.035). CONCLUSIONS: The probability of unilateral umbilical artery with heart malformation is increased. SUA complicated with cardiac congenital malformations is most common in conotruncal arteriole and ventricular septal defect. SUA fetal left umbilical cord loss ratio is greater than the right side of the right umbilical artery missing fetus more likely to merge with heart malformations.
