目的:探讨早期安氏Ⅲ类错的良好矫治方法。方法:将到我科门诊就诊的100例早期安氏Ⅲ类错的病例随机分为两组,A组50例采用传统垫矫治器进行矫治,B组50例用改良式垫矫治器进行矫治,两组进行对比观察,分析矫治前后的软硬组织变化,评价治疗效果。结果:矫治后两组上下颌骨位置及软组织均有改善。B组上下中切牙角由矫治前至矫治后减少12.0°(P<0.01);SNA角由矫治前至矫治后增大2.4°(P<0.05);SNB角由矫治前至矫治后减少2.1°(P<0.01);ANB角由矫治前负值至矫治后达到正值,增加4.4°(P<0.05)。两组矫治后比较,SNB角、ANB角和L1-NB、L1-NB角B组优于A组(P<0.05)。结论:改良式垫矫治器同时具有机械及功能矫治的双重作用,可以同时改善上下颌骨的位置关系,下颌后退效果更明显,牙颌面关系更协调。
Objective: To explore a good correction method for early Class Ⅲ malocclusion. Methods: A total of 100 cases of early Class Ⅲ malocclusion in our outpatient department were randomly divided into two groups. 50 cases in group A were treated with traditional otolaryngoscope, 50 cases in group B were treated with modified pad appliance The two groups were compared, the changes of the soft and hard tissues before and after the treatment were analyzed, and the treatment effect was evaluated. Results: The position of mandible and soft tissue in both groups improved after the treatment. The angle of upper and lower incisors in group B decreased by 12.0 ° (P <0.01) before and after treatment, while the SNA angle increased by 2.4 ° (2.4%, P <0.05) before and after treatment. The SNB angle decreased by 2.1 ° (P <0.01). The ANB angle reached a positive value from the negative value before treatment to a positive value of 4.4 ° (P <0.05). After treatment, SNB angle, ANB angle, L1-NB and L1-NB angle in group B were better than those in group A (P <0.05). Conclusion: The modified pad appliance has the dual functions of mechanical and functional correction, which can improve the position of the mandible at the same time. The effect of mandibular retraction is more obvious and the dento-maxillofacial relationship is more harmonious.
