目的:分析178例行人工耳蜗植入患者的手术情况、术中出现的问题及应对措施,以提高人工耳蜗植入效率及回避并发症。方法:选取2012年9月—2016年5月在我院行人工耳蜗植入的178例患者,对术中出现的问题及应对措施进行临床分析。结果:178例均一期完成人工耳蜗植入术。术中面临的主要问题为:圆窗龛暴露不良38例,井喷现象14例,乳突轮廓化不良18例,圆窗龛标志不清2例。同一患者可能同时伴有上述多个问题,术后均未发现严重并发症。结论:(1)人工耳蜗植入手术需要术者扎实的耳科解剖学基础,并具备良好的影像阅片能力及手术技巧;(2)不同患者出现的手术问题需采取个性化处理方式解决。
OBJECTIVE: To analyze the operative conditions, intraoperative problems and countermeasures in 178 patients undergoing cochlear implantation to improve the cochlear implantation efficiency and avoid complications. Methods: A total of 178 patients undergoing cochlear implantation in our hospital from September 2012 to May 2016 were selected for clinical analysis of the intraoperative problems and countermeasures. Results: All 178 patients underwent cochlear implantation. The main problems in operation were: round window niche exposure in 38 cases, blowout phenomenon in 14 cases, mastoid contouring in 18 cases, circular window niche sign in 2 cases. The same patient may be accompanied by many of these problems, no serious complications were found after surgery. Conclusion: (1) cochlear implant surgery requires a solid anatomy of the otologist, and have good imaging capabilities and surgical skills; (2) surgical problems in different patients need to take a personalized approach to solve.
