目的评价MRI在胎儿中枢神经系统畸形中的应用价值。资料与方法 22例孕妇,年龄23~40岁,年龄平均28岁;孕龄23~40周,平均30周。产前超声(US)检查后48 h内行MRI检查,T2WI采用半傅里叶单激发快速自旋回波序列(HASTE)、真稳态进动快速成像序列(True FISP),T1WI采用超快速小角度激发序列(FLASH)(Turbo FLASH)扫描,将产前MRI、US表现与产后诊断或尸检结果对照。结果产前MRI与US诊断一致并正确者9例,产前MRI正确并证实US可疑病变2例,产前MRI正确并完善产前US诊断1例,产前MRI正确并改变产前US诊断7例,产前US正确MRI不正确1例,产前US正确且完善MRI诊断2例。结论 MRI对胎儿神经系统畸形的诊断具有一定的优势,与US相比,MRI可提供更全面、更准确的信息,甚至更正US诊断,可作为胎儿中枢神经系统畸形的一种重要检查方法和补充手段。
Objective To evaluate the value of MRI in the diagnosis of fetal central nervous system deformity. Materials and Methods 22 pregnant women, aged 23 to 40 years old, mean age 28 years; gestational age 23 to 40 weeks, an average of 30 weeks. MRI was performed within 48 hours after prenatal ultrasound (US) examination. T2WI was performed using HASTE, True FISP, and T1WI with ultra-fast and low-angle FLASH (Turbo FLASH) scan, the prenatal MRI, US performance and postpartum diagnosis or autopsy results. Results The prenatal MRI was consistent with the US diagnosis in 9 cases. The prenatal MRI was correct and confirmed the suspicious lesions in 2 cases. The prenatal MRI was correct and the prenatal US diagnosis was completed in 1 case. The prenatal MRI was correct and the prenatal US diagnosis was changed. Cases, prenatal US correct correct MRI in 1 case, prenatal US correct and perfect MRI diagnosis in 2 cases. Conclusion MRI has certain advantages in the diagnosis of fetal neurological deformity. Compared with US, MRI can provide more comprehensive and accurate information, and even correct the US diagnosis, which can be used as an important examination method and supplement for fetal central nervous system deformity means.
