目的:评估巢氏-PCR和ELISA在巨细胞病毒(HCMV)感染筛查诊断中的价值。方法:以巢式PCR检测新生儿HCMV-DNA为标准,评估ELISA方法检测HCMV-IgG和IgM的诊断价值。结果:采用巢氏PCR方法,患病新生儿HC-MV-DNA检出率为63.900%。以巢式PCR为标准,ELISA-IgM法检测HCMV的灵敏度为3.030%,特异度为100.000%,阳性预测值为100.000%,阴性预测值为29.670%;ELISA-IgG法检测HCMV的灵敏度为36.620%,特异度为42.110%,阳性预测值为70.270%,阴性预测值为15.090%。结论:巢式PCR是筛检HCMV感染的灵敏方法,目前市场上ELISA试剂盒的灵敏度有待提高。
Objective: To evaluate the value of nested PCR and ELISA in the screening of cytomegalovirus (HCMV) infection. Methods: Neonatal HCMV-DNA was detected by nested PCR as a standard to evaluate the diagnostic value of ELISA for detection of HCMV-IgG and IgM. Results: The detection rate of HC-MV-DNA in sick newborns was 63.900% by nested PCR. The nested PCR as the standard, the sensitivity of HCMV by ELISA-IgM was 3.030%, the specificity was 100.000%, the positive predictive value was 100.000%, the negative predictive value was 29.670%; the sensitivity of ELISA-IgG method for detecting HCMV was 36.620% , The specificity was 42.110%, the positive predictive value was 70.270% and the negative predictive value was 15.090%. Conclusion: Nested PCR is a sensitive method for screening for HCMV infection. The sensitivity of ELISA kit on the market needs to be improved at present.
