目的了解如东县医疗机构消毒质量状况,有效地预防和控制医院感染的发生。方法采用现场采样实验室检验的方法,对辖区内各级医疗机构不同消毒灭菌处理对象的消毒灭菌效果进行监测。结果 2011-2013年共采样3 984份,总合格率为95.3%,3年的合格率分别为91.7%、95.4%和98.5%,合格率差异有统计学意义,χ2=66.3,P<0.05。不同的检测项目合格率分别为室内空气86.1%、物体表面97.2%、医护人员手91.8%、使用中消毒剂99.0%、压力蒸汽灭菌效果95.4%、一次性医疗卫生用品100.0%、紫外灯辐射强度94.3%、污水92.3%,不同的检测项目合格率差异有统计学意义,χ2=90.25,P<0.05。不同级别医疗机构合格率分别为中心级及以上98.0%、民营医院98.8%、社区卫生服务站93.4%,差异有统计学意义,χ2=55.7,P<0.05。结论如东县各级医疗机构消毒工作状况总体较好,样品监测合格率高,但消毒质量仍存在问题,需进一步加强消毒监测和管理。
Objective To understand the quality of disinfection in medical institutions in Rudong County and to effectively prevent and control the occurrence of nosocomial infections. Methods The method of on-site sampling laboratory was used to monitor the disinfection and sterilization effects of different disinfection and sterilization objects in medical institutions at all levels. Results A total of 3 984 samples were obtained during the period of 2011-2013, with a total pass rate of 95.3%. The three-year pass rates were 91.7%, 95.4% and 98.5% respectively. The difference in pass rates was statistically significant (χ2 = 66.3, P <0.05). The qualified rates of different test items were 86.1% of indoor air, 97.2% of object surface, 91.8% of medical staff, 99.0% of disinfectant in use, 95.4% of steam sterilization, 100.0% of disposable medical and sanitary products, The intensity was 94.3% and the sewage was 92.3%. There was a significant difference in the passing rates of different testing items, χ2 = 90.25, P <0.05. The qualification rates of medical institutions at different levels were 98.0% at the center level and above, 98.8% at private hospitals and 93.4% at community health service stations respectively. The difference was statistically significant (χ2 = 55.7, P <0.05). Conclusion The disinfection work of medical institutions at all levels in Rudong is generally good and the qualified rate of sample monitoring is high. However, there are still some problems in the quality of disinfection, and the disinfection monitoring and management needs to be further strengthened.
