目的探讨瑞芬太尼复合七氟醚在肝功能障碍婴儿手术中的应用。方法婴儿先天性胆总管囊肿手术40例,随机分为两组,瑞芬太尼复合七氟醚维持麻醉组(R组)和单纯七氟醚维持麻醉组(S组)。记录诱导前(T0)、切皮前(T1)、切皮后10 min(T2)、手术开始60 min(T3)、缝皮(T4)时的MAP、HR、CETSev。记录手术时间、呼吸恢复时间、拔管时间、Aldrete评分达10分时的清醒时间。结果 T1~T4时R组、T1~T3时S组HR、MAP明显低于T0时(<0.05);T2~T4时R组HR、MAP明显低于S组(<0.05);T2~T4时R组CETSev均明显低于S组(0.05);R组呼吸恢复时间、拔管时间、Aldrete评分达10分时的清醒时间均明显低于S组(<0.05)。结论在肝功能障碍婴儿手术中使用瑞芬太尼复合七氟醚维持麻醉,术中麻醉效果好、循环稳定,且患儿术后苏醒迅速。因此瑞芬太尼复合七氟醚适用于肝功能障碍的婴儿的麻醉维持。“,”Objective Discussion on the application of remifentanil and sevoflurane in liver dysfunction operation in infants . Methods Infants with congenital choledochal cyst operation in 40 cases, were randomly divided into two groups, remifentanil and sevoflurane to maintain anesthesia group (group R) and simple anesthesia maintenance group (group S).MAP、 HR、 CETSev were abserved before induction (T0), before skin incision (T1), 10 min after skin incision (T2), 60 min after operation beginning (T3), sewing leather (T4) . The operation time, breathing recovery time, extubation time, awake time of Aldrete score of 10 points were recorded. Results In group R, HR and MAP were lower at T1~T4 than that at T0. In group S, HR and MAP were lower at T1~T3 than that at T0 ( <0.05). HR and MAP were lower at T2~T4 than those in group S ( <0.05). In group R, CETSev was lower at T2~T4 than those in group S ( 0.05). In group R , breathing recovery time、extubation time awake time of Aldrete score of 10 points were lower than those in group S ( <0.05). Conclusion Remifentanil and sevoflurane in liver dysfunction in infants operation can provide satisfactory anesthetic ef ect. The children quickly regained consciousness after the surgery.Therefore, Remifentainil and sevoflurane is suitable for the infant of liver function obstacle .
