目的:分析肝硬化并低氧血症患者的发生率、发病机理,探讨有效的临床治疗方法。方法:选取我院2015年12月-2016年12月收治的50例肝硬化患者作为观察组,同期在我院体检的50例健康体检人员作为对照组,采集所有受检者的晨起空腹静脉血标本利用自动血气分析仪进行PH值、氧饱和度(SaO_2)、二氧化碳分压(PaCO_2)、血氧分压(PaO_2)检测,对比观察两组检测结果,记录观察组患者低氧血症发生率,分析肝硬化患者发生低氧血症的发病机制。结果:(1)观察组中有26例患者出现低氧血症,发生率为52.50%;(2)观察组PaO_2、SaO_2值明显低于对照组,两组数据对比具有统计学意义P<0.05;(3)两组PH值、PaCO_2无显著性差异(P<0.05);(4)观察组中肝功能Child C级患者的PaO_2、SaO_2值明显低于Child B级、Child A级患者(P<0.05)。结论:肝硬化患者发生低氧血症的概率高,主要与门静脉高压、肝功能缺陷、蜘蛛痣、肺血管异常、睡眠质量下降等因素有关,对患者进行PaO_2、SaO_2检测可以预测低氧血症发生率,评估患者预后,为治疗方案的选择提供诊断依据。
Objective: To analyze the incidence of cirrhosis and hypoxemia in patients with pathogenesis, to explore effective clinical treatment. Methods: Fifty patients with cirrhosis admitted from December 2015 to December 2016 in our hospital were selected as the observation group. Fifty healthy physical examination staff in our hospital during the same period were selected as the control group. All the subjects underwent early fasting veins The blood samples were detected by the automatic blood gas analyzer for PH value, oxygen saturation (SaO_2), partial pressure of carbon dioxide (PaCO_2) and partial pressure of oxygen (PaO_2). The results of two groups were compared and the hypoxemia Rate, analysis of pathogenesis of hypoxemia in patients with cirrhosis. Results: (1) Hypoxemia occurred in 26 patients (52.50%) in the observation group; (2) PaO_2 and SaO_2 values in the observation group were significantly lower than those in the control group (P <0.05) ; (3) There was no significant difference in PaCO_2 between the two groups (P <0.05); (4) PaO_2 and SaO_2 values in Child C grade of liver function in observation group were significantly lower than those in Child B and Child A <0.05). Conclusion: The incidence of hypoxemia in patients with cirrhosis is high, mainly related to the factors such as portal hypertension, hepatic dysfunction, spider nevus, pulmonary vascular abnormalities and decreased sleep quality. PaO_2 and SaO_2 detection in patients with cirrhosis can predict hypoxemia Incidence, assessment of patient prognosis, to provide a diagnostic basis for the choice of treatment options.
