目的:探讨脊髓栓系综合征(TCS,Tethered cord syndrome)的MRI表现及其在TCS中的应用价值。方法:回顾性分析39例TCS患者的MRI表现及特点。结果:全部患者圆锥尖部位置低于L3椎体中部以下,脊髓受牵拉固定。39例TCS患者中,脊髓膨出及脊髓脊膜膨出或脂肪脊髓脊膜膨出18例,椎管内脂肪瘤或脂肪沉积14例,脊髓终丝增粗15例,脊髓纵裂8例,脊髓空洞或软化灶7例,脊柱侧弯半椎及融合畸形11例,皮样囊肿4例,合并畸胎瘤1例。结论:MRI是诊断脊髓栓系综合征首选检查方法。
Objective: To investigate the MRI findings of tethered cord syndrome (TCS) and its application in TCS. Methods: MRI findings and features of 39 patients with TCS were analyzed retrospectively. Results: The position of conus tip in all patients was lower than that in the middle of L3 vertebral body, and the spinal cord was pulled and fixed. Among the 39 patients with TCS, 18 cases had spondylolisthesis and myelomeningocele or myelomeningocele, 14 cases of intubation of lipoma or fat deposits, 15 cases of spinal cord terminal thickening, 8 cases of spinal cord rupture, Syringomyelia or softening in 7 cases, scoliosis hemifasion and fusion deformity in 11 cases, dermoid cysts in 4 cases, 1 case of teratoma. Conclusion: MRI is the first choice for the diagnosis of tethered cord syndrome.
