目的 探索椎管肿瘤采用显微外科技术摘除的疗效。 方法 自 1990年以来 ,共有 2 3例椎管肿瘤致程度不同截瘫病例 ,对其致瘫原因进行分析 ,同时提出采用显微外科技术切除肿瘤的方法与优、缺点。 结果 经术后 3~ 2 6个月随访 ,依据美国脊柱损伤学会对Frankel脊髓损伤修订分级标准评定 :除 2例术后截瘫未完全恢复外 ,其它 2 1例瘫痪完全恢复。 结论 采用显微外科技术切除椎管肿瘤 ,具有解剖层次分明、组织分辨率高 ,肿瘤切除完全、残留肿瘤少 ,对脊髓、神经根干扰降低到最低限度 ,其手术效果明显优于普通手术切除。
Objective To explore the curative effect of microsurgical removal of spinal canal tumors. Methods Since 1990, a total of 23 cases of paraplegia caused by spinal tumors have been analyzed. The causes of paralysis were analyzed. At the same time, the methods and advantages and disadvantages of using microsurgical techniques to remove tumors were proposed. Results According to the American Society of Spine Injury, Frankel’s criteria for revision of the spinal cord injury were evaluated. Among the 2 cases, paraplegia was not completely recovered after operation, and 21 cases were completely paralyzed. Conclusions Microsurgery is an effective method to remove the tumor of the spinal canal. It has distinct levels of anatomy, high tissue resolution, complete tumor resection, few residual tumors and minimal interference with the spinal cord and nerve root. The surgical results are obviously better than those of ordinary surgical resection.
