老年人因生理生化过程发生变化,体内药物动力学过程与青年人相比:药物吸收速率和血药峰浓度下降;吸收半衰期和达峰时间延长;生物利用度变化不明显。长期用药的慢性病患者应根据体内药量变化适当改变剂量。影响老年人药物代谢的因素很多,但一般是代谢功能降低,导致游离药物浓度增加。因肾小球滤过率和肾小管分泌作用减慢,致使药物的作用强度和持续时间都超过青年人。故对老年人用药问题更应引起重视,更需要设计个体化的给药方案。
Elderly due to changes in physiological and biochemical processes, pharmacokinetics in vivo compared with young people: drug absorption rate and peak plasma concentration decreased; absorption half-life and peak time; bioavailability was not significantly changed. Long-term medication in patients with chronic diseases should be based on changes in the dose of the body appropriate to change the dose. Many factors affect the metabolism of the elderly drug, but generally reduced metabolic function, resulting in increased free drug concentration. Due to glomerular filtration rate and tubular secretion slowed down, resulting in the intensity and duration of the drug than young people. Therefore, the issue of medication for the elderly should pay more attention, but also need to design individualized dosing regimen.
