目的 研究外周及甲状腺静脉血中白介素 6 (IL 6 )浓度的差异 ,探讨IL 6在甲状腺稳态和甲状腺疾病发病机理中的作用与意义。方法 于手术过程中同时采集Graves’病 (GD)和非自身免疫性甲状腺疾病 (AITD)患者甲状腺静脉血和外周静脉血。以超敏ELISA方法 ,测定血样本IL 6的含量。结果 接受手术的 6例GD和 1 5例非AITD患者外周静脉血中IL 6水平与正常对照比较无统计学差异 ,但两组患者甲状腺内静脉血IL 6浓度均明显高于其外周静脉血。所有患者两种血样本IL 6含量与甲状腺激素水平无显著相关性。GD组甲状腺内血IL 6与促甲状腺激素受体抗体 (TRAb)活性显著相关 ,而GD外周血及非AITD甲状腺内、外血与TRAb无关。结论 甲状腺组织能够合成与分泌高浓度的IL 6 ,后者可作为甲状腺破坏的观察指标 ,并在AITD的发展中发挥重要作用。
Objective To study the difference of interleukin 6 (IL 6) concentration in peripheral and thyroid venous blood and explore the role and significance of IL 6 in the pathogenesis of thyroid homeostasis and thyroid disease. Methods Both thyroid venous blood and peripheral venous blood of patients with Graves’ disease (GD) and non-autoimmune thyroid disease (AITD) were collected during the operation. Using a hypersensitive ELISA method, the amount of IL 6 in the blood sample was determined. Results Six cases of GD and 15 cases of non-AITD had no significant difference in peripheral venous blood IL-6 levels compared with the control group. However, the levels of IL-6 in the two groups were significantly higher than those in the peripheral venous blood. There was no significant correlation between IL 6 levels and thyroid hormone levels in the two blood samples from all patients. The levels of IL 6 and thyrotropin receptor antibody (TRAb) in peripheral blood of GD patients were significantly correlated with those of peripheral blood of GD patients and non-AITD thyroid patients. Conclusion Thyroid tissue can synthesize and secrete high concentration of IL 6, which can be used as an indicator of thyroid destruction and play an important role in the development of AITD.
