目的探讨下肢肿瘤放疗中利用丁字鞋固定体位相较于常规负压垫固定对摆位时间和摆位精度的影响。方法选择下肢肿瘤患者20例,简单随机分为A组(10例)和B组(10例),A组用真空负压垫固定,B组用丁字鞋定位,每例患者治疗前均采用锥形束CT(cone beam CT,CBCT)扫描配准纠正摆位误差,统计每例患者的摆位误差和摆位时间,并对2组数据进行统计分析。结果丁字鞋固定组摆位时间明显低于负压垫固定组的摆位时间(P<0.05);CBCT图像配准结果中,丁字鞋固定组在Lat左右和Lng前后方向上均优于负压垫固定组[(0.25±0.19 vs 0.44±0.16)cm,P=0.028;(0.27±0.18 vs 0.58±0.13)cm,P<0.01]。结论下肢肿瘤放疗定位采用丁字鞋定位技术可以减小摆位难度,缩短摆位时间,减少摆位误差,值得在临床中推荐使用。
Objective To investigate the effect of fixed position of T-shaped shoes on radiotherapy of lower extremity tumors compared with conventional negative pressure pad fixation on setting time and positioning accuracy. Methods Twenty patients with lower extremity tumors were selected and randomly divided into group A (n = 10) and group B (n = 10). Group A was fixed with a vacuum negative pressure pad and group B with a toe shoes. Before each treatment, cone The registration of beamform CT (CBCT) was used to correct the setup error, and the setup error and setup time of each patient were calculated. Two groups of data were statistically analyzed. Results The setting time of T-shoe fixation group was significantly lower than that of the negative pressure pad fixation group (P <0.05). In CBCT image registration, the T-shaped shoe fixation group was superior to negative pressure Pad fixation group [(0.25 ± 0.19 vs 0.44 ± 0.16) cm, P = 0.028; (0.27 ± 0.18 vs 0.58 ± 0.13) cm, P <0.01]. Conclusion Radiotherapy positioning of lower extremity tumors using T-shaped shoes positioning technology can reduce the difficulty of positioning, shorten the setting time and reduce the setting error, which is worth recommending in clinic.
