目的探讨结肠癌转移相关基因1(MACC1)和抑癌基因KISS-1在浸润性乳腺癌(IBC)中的蛋白表达及其与上皮间质转化相关因子Snail表达的关系和临床病理意义。方法通过免疫组织化学法检测250例IBC(IBC组)和80例正常乳腺组织(对照组)中MACC1、KISS-1和Snail的表达情况,分析临床病理资料特征,评估对患者预后的影响。结果在对照组中,MACC1、KISS-1和Snail的阳性表达率分别为7.5%、87.5%和5.0%;在IBC组中,MACC1、KISS-1和Snail阳性表达率分别为63.6%、38.0%和58.8%,两组之间差异均有统计学意义(P<0.05)。MACC1、KISS-1和Snail蛋白的表达在IBC患者的淋巴结转移与否、肿瘤组织的不同分化程度、不同临床分期、不同分型之间差异均有统计学意义(P<0.05)。MACC1和Snail蛋白阳性表达组患者总的生存时间低于其阴性表达组患者(P<0.001);KISS-1表达阳性的患者其生存时间高于其阴性组患者(P<0.001)。Cox回归分析结果表明,MACC1、Snail蛋白的阳性表达和KISS-1的阴性表达是影响IBC患者术后生存时间的独立危险因素(P<0.05)。结论MACC1、Snail和KISS-1的反常表达参与了IBC的浸润及转移过程,早期联合检测MACC1、Snail和KISS-1的表达对IBC的进展及预后判定均有重要意义。
Objective To investigate the relationship between the expression of MACC1 and KISS-1 in invasive breast cancer (IBC) and the expression of Snail and its clinicopathological significance. Methods The expressions of MACC1, KISS-1 and Snail in 250 cases of IBC (IBC group) and 80 cases of normal breast tissue (control group) were detected by immunohistochemistry. The clinical and pathological features were analyzed to evaluate the prognosis of patients. Results In the control group, the positive rates of MACC1, KISS-1 and Snail were 7.5%, 87.5% and 5.0%, respectively. The positive rates of MACC1, KISS-1 and Snail in IBC were 63.6% and 38.0% And 58.8% respectively. There was significant difference between the two groups (P <0.05). The expression of MACC1, KISS-1 and Snail protein in IBC patients with lymph node metastasis or not, tumor tissue differentiation, clinical stage, different types of differences were statistically significant (P <0.05). The overall survival time of MACC1 and Snail positive patients was lower than that of negative patients (P <0.001). The survival time of patients with positive KISS-1 expression was higher than that of negative patients (P <0.001). Cox regression analysis showed that the positive expression of MACC1 and Snail protein and the negative expression of KISS-1 were the independent risk factors affecting the survival time of IBC patients (P <0.05). Conclusions The abnormal expression of MACC1, Snail and KISS-1 is involved in the infiltration and metastasis of IBC. The early detection of the expression of MACC1, Snail and KISS-1 is of great significance for the progression and prognosis of IBC.
