目的比较不同促排卵方案对不明原因不孕患者宫腔内人工授精(IUI)周期的影响。方法回顾性分析2007年1月至2015年12月,北京市海淀区妇幼保健院生殖医学中心施行IUI的不明原因患者资料,比较自然周期组、克罗米芬(CC)组、来曲唑(LE)组、CC+HMG组和LE+HMG组行IUI的妊娠情况。结果 355对不明原因不孕夫妇行713周期宫腔内人工授精,临床妊娠80周期,临床妊娠率11.22%。各组间年龄、不孕时间、不孕类型和洗精后前向运动精子总数差异无统计学意义(F=1.031、F=1.627、χ2=6.501、Z=-1.645,P均>0.05)。CC、LE、CC+HMG和LE+HMG组平均排卵数均高于自然周期组,差异均有统计学意义(Z=-11.636、-8.057、-8.258、-11.497,P均=0.000),CC组、LE组和LE+HMG组HCG日子宫内膜厚度低于自然周期组,差异均有统计学意义(t=7.529、4.022、4.656,P均=0.000)。CC组妊娠率最高为15.29%,自然周期组妊娠率8.80%,二组比较差异有统计学意义(χ2=4.74,P=0.029)。结论不明原因不孕患者在IUI周期中采用克罗米芬促排卵,妊娠率更高。
Objective To compare the effect of different ovulation induction programs on intrauterine insemination (IUI) cycles in patients with unexplained infertility. Methods The data of patients with unexplained causes of IUI at the Reproductive Medicine Center of MCH Hospital of Haidian District, Beijing from January 2007 to December 2015 were retrospectively analyzed. The data of natural period group, clomiphene citrate group, letrozole (LE) Group, CC + HMG group and LE + HMG group IUI pregnancy. Results 355 couples with unexplained infertility performed 713 cycles of intrauterine insemination, 80 cycles of clinical pregnancy and clinical pregnancy rate of 11.22%. There were no significant differences in the age, infertility time, type of infertility and the total number of forward motile sperm after washing (F = 1.031, F = 1.627, χ2 = 6.501, Z = -1.645, P> 0.05). The average number of ovulation in the CC, LE, CC + HMG and LE + HMG groups was significantly higher than that of the natural cycle group (Z = -11.636, -8.057, -8.258, -11.497, P = 0.000) The endometrial thickness of HCG group in LE group and LE + HMG group was lower than that in natural cycle group (t = 7.529,4.022,4.656, P = 0.000). The highest pregnancy rate was 15.29% in CC group and 8.80% in spontaneous cycle group. There was significant difference between the two groups (χ2 = 4.74, P = 0.029). Conclusions In unexplained infertility patients, clomiphene is used to promote ovulation in IUI cycle, and the pregnancy rate is higher.
