目的观察2型糖尿病并脑梗死患者血浆溶血磷脂酸(lysophosphatidic acid,LPA)、空腹血糖(fasting plasma glucose,FPG)、糖化血红蛋白(glycosylated hemoglobin A1c,HbA1C)水平变化,探讨LPA水平与2型糖尿病及大血管病变的关系。方法 2型糖尿病并急性脑梗死患者(观察组)42例、非糖尿病急性脑梗死患者(脑梗死组)47例、2型糖尿病且无糖尿病急慢性并发症患者(糖尿病组)41例、门诊体检健康者(对照组)46例,检测4组血浆LPA,FPG及HbA1C水平;观察组和脑梗死组于入院7d后再次检测血浆LPA水平。结果观察组、脑梗死组和糖尿病组LPA水平均高于对照组(P<0.01);观察组LPA,FPG及HbA1C水平均高于脑梗死组、糖尿病组(P<0.01);发病7d后,观察组和脑梗死组LPA水平均较入院时下降,但观察组下降幅度小于脑梗死组(P<0.01)。结论超负荷血糖可能是2型糖尿病患者LPA水平增高的一个重要因素,2型糖尿病引发缺血性脑血管病形成可能部分与血浆LPA水平增高有关。
Objective To observe the plasma levels of lysophosphatidic acid (LPA), fasting plasma glucose (FPG) and glycosylated hemoglobin A1c (HbA1C) in patients with type 2 diabetes mellitus and cerebral infarction. To investigate the relationship between LPA levels and type 2 diabetes mellitus The relationship between macrovascular disease. Methods Forty-two patients with type 2 diabetes mellitus and acute cerebral infarction (observation group), 47 patients with non-diabetic acute cerebral infarction (cerebral infarction group), 41 type 2 diabetic patients without diabetes mellitus (diabetic group) Forty-six healthy subjects (control group) were enrolled in this study. Plasma LPA, FPG and HbA1C levels were measured in four groups. LPA levels in the observation and cerebral infarction groups were measured again 7 days after admission. Results The levels of LPA, FPG and HbA1C in observation group, cerebral infarction group and diabetic group were significantly higher than those in control group (P <0.01). The levels of LPA, FPG and HbA1C in observation group were higher than those in cerebral infarction group and diabetes group (P <0.01) The levels of LPA in observation group and cerebral infarction group were lower than those in admission group, but the decreasing rate in observation group was smaller than that in cerebral infarction group (P <0.01). Conclusion Hyperglycemia may be an important factor in the increase of LPA in patients with type 2 diabetes. The formation of ischemic cerebrovascular disease may be partly related to the increase of LPA in type 2 diabetes mellitus.
