目前,影响穿透角膜移植成功率的主要因素是移植免疫反应和移植手术操作。手术操作又与手术方式密切相连。由于手术显微镜的改进,手术方式多为显微镜下直接缝合角膜移植片,并已取得较好的效果。但是对于需要用较小(如5mm直径)的移植片进行移植的患者,直接缝合就很不适宜。既往曾有使用间接缝线及各种盖片(如塑料盘、生物膜)等多种间接固定角膜移植片的手术方法,但是均有固定不牢、合并症多等缺点。因而寻找既不损伤移植片、又能固定牢固,以及方法简单
At present, the main factors that affect the success rate of penetrating keratoplasty are the transplant immune response and transplant operation. Surgical procedures and surgical methods are closely linked. Due to the improvement of surgical microscope, surgical methods are mostly direct suture corneal graft under the microscope, and have achieved good results. However, for patients who need to be implanted with a smaller (eg, 5mm diameter) graft, direct suture is not appropriate. There have been a number of indirect methods of using indirect sutures and various covers (such as plastic trays and biofilms) for indirect fixation of corneal grafts.
