目的:探讨胰腺神经内分泌肿瘤(NETP)的MSCT影像表现。方法:回顾性分析经手术病理证实的9例功能性NETP、12例非功能性NETP的MSCT征象,包括肿瘤病灶发生的部位、大小、密度及强化特征。结果:功能性NETP较小,长径约0.9~2.3 cm,平均约1.4 cm,非功能性NETP长径约2.1~6.4 cm,平均约4.3 cm。肿瘤可发生于胰腺任何部位,体尾部略多,功能性NETP发生于胰头2例、胰体3例、胰尾4例,非功能性NETP发生于胰头3例、胰体3例、胰尾6例。功能性NETP平扫病灶呈等密度3例,低密度6例;非功能性NETP平扫病灶呈低密度12例,病灶内囊变、坏死7例,病灶内见点状钙化2例。增强扫描功能性及非功能性NETP动脉期病灶实质部分均呈中度至明显强化,静脉期强化程度减低,呈“快进、快出”改变,6例病灶表现为均匀强化,4例非功能性NETP表现为环形强化。结论:胰腺神经内分泌肿瘤MSCT具有一定的特征性表现,有助于肿瘤的鉴别及指导临床治疗。
Objective: To investigate the MSCT features of pancreatic neuroendocrine neoplasms (NETP). Methods: We retrospectively analyzed the MSCT signs of 9 cases of functional NETP and 12 cases of nonfunctioning NETP confirmed by pathology, including the location, size, density and enhancement characteristics of tumor lesions. RESULTS: Functional NETP was small, with a long diameter of about 0.9-2.3 cm and an average of about 1.4 cm. The non-functional NETP had a long diameter of about 2.1-6.4 cm and an average of about 4.3 cm. Tumors can occur in any part of the pancreas with slightly more body and tail. Functional NETP occurred in 2 cases of pancreas, 3 cases of pancreas, and 4 cases of pancreas. Non-functioning NETP occurred in 3 cases of pancreas, 3 cases of pancreas, and pancreas. Tail 6 cases. Functional NETP plain scan lesions were of equal density in 3 cases, low density in 6 cases; non-functional NETP edema lesions showed low density in 12 cases, lesions in the cystic degeneration, necrosis in 7 cases, lesions in the spot calcification in 2 cases. The enhancement of functional scans and non-functional NETP arterial phase lesions showed moderate to marked enhancement in the parenchymal part, reduced venous phase enhancement, showed “fast-forward, fast-out” changes, 6 lesions showed homogeneous enhancement, 4 cases Non-functional NETPs exhibit ring enhancement. Conclusion: The MSCT of pancreatic neuroendocrine tumors has some characteristic features, which is helpful for the identification of tumors and clinical treatment.
