目的探讨促性腺激素释放激素(LHRH)刺激试验在鉴别性早熟类型中的价值。方法 37例性早熟女性儿童,行LHRH激发试验,检测基础血清雌二醇(E2)、骨龄、子宫及卵巢容积,并随访。结果 37例中,真性(中枢性)性早熟12例(CPP组),假性(外周性)性早熟25例(PPP组);经LHRH刺激试验后,CPP组与PPP组黄体生成素(LH)、卵泡刺激素(FSH)峰值均较基础值升高,其中CPP组LH峰值较PPP组LH的峰值升高明显[(21.02±10.17)IU/Lvs.(6.17±2.41)IU/L,(P<0.01)];FSH峰值差异无统计学意义。两组患者骨龄均有部分提前,CPP组与PPP组骨龄指数(BAI)差异有统计学意义[(1.14±0.09)vs.(1.04±0.10),(P<0.05)]。两组基础E2值、子宫、卵巢容积差异无统计学意义。结论 LHRH刺激试验对鉴别性早熟类型有临床意义。
Objective To investigate the value of gonadotropin-releasing hormone (LHRH) stimulation test in differentiating precocious types. Methods 37 cases of precocious female children underwent LHRH provocation test, and the basal serum estradiol (E2), bone age, uterine and ovarian volume were measured and followed up. Results In 37 cases, there were 12 cases of true precocious puberty (CPP group) and 25 cases of pseudo (peripheral) precocious puberty. After LHRH stimulation test, the levels of luteinizing hormone (P <0.05). The peak values of follicle stimulating hormone (FSH) were higher than those of basal group, and the peak value of LH in CPP group was significantly higher than that in PPP group [(21.02 ± 10.17) IU / L vs P <0.01)]; FSH peak difference was not statistically significant. Bone age was advanced in both groups. The bone age index (BAI) in CPP group and PPP group was significantly different ([1.14 ± 0.09] vs. (1.04 ± 0.10), P <0.05]. There was no significant difference in basal E2 value, uterine and ovarian volume between the two groups. Conclusions LHRH stimulation test has clinical significance for the type of precocious puberty.
