目的:探讨超声波检查在婴幼儿髋关节脱位治疗中的监测作用。方法:回顾性分析2011年1月-2013年12月在重庆市合川区人民医院接受治疗的80例髋关节脱位婴幼儿的超声诊断资料,其中,双侧髋关节脱位36例,左侧髋关节脱位28例,右侧髋关节脱位16例,共116髋。Graf分型:Ⅱc型62髋,Ⅱd型31髋,Ⅲ型17髋,Ⅳ型为6髋,对本组116髋均行髋关节吊带治疗,在超声波监测下调整吊带位置,观察吊带治疗后的吊带安装随访结果和髋关节治疗情况。结果:本组80例患儿共116髋经吊带治疗后,8髋治疗失败,为治疗两周后家属放弃吊带治疗,其余108髋吊带治疗后3周经超声波检查,髋关节基本保持中心性复位,其中,102髋安装吊带后一次复位,6髋在超声波监测下调整吊带位置最终复位满意,所有患儿在巩固治疗7~9周之后,超声波检查显示髋臼骨顶角值大于60°,髋关节完全复位,所有患儿均未发生股骨头缺血性坏死。结论:在髋关节脱位的临床治疗中,超声波检查是一种安全、简便、有效的诊断方法,尤其在吊带治疗过程中,借助超声波检查,可监测吊带治疗情况,便于主治医生及时调整吊带,更改治疗方案,最终达到满意复位的治疗效果。
Objective: To investigate the monitoring effect of ultrasound in the treatment of hip dislocation in infants. Conclusion: In the clinical treatment of dislocation of the hip, ultrasonic examination is a safe, simple and effective method of diagnosis. Especially in the process of sling treatment, ultrasonic examination can monitor the treatment of the sling so that the attending surgeon can timely adjust the sling to change Treatment program, and ultimately achieve the desired reduction in the treatment effect.
