目的 观察和探讨抗原表达异常的急性髓细胞性白血病 (AML)患者端粒酶活性及对预后的影响。方法 采用端粒重复序列聚和酶链反应扩增法 (TRAP PCR)检测 30例AML患者骨髓的端粒酶活性。结果 30例AML患者端粒酶阳性者 1 5例 ,阳性率为 50 %。Ly+ 组 (凡细胞既有髓系抗原表达 ,又有一种或两种以上淋系抗原呈阳性反应 ,阳性率≥ 30 %为Ly+ AML)与Ly- 组 (凡细胞仅表达髓系抗原 ,不伴有淋系抗原表达者为Ly- AML)端粒酶阳性率不一致。Ly+ 组较Ly- 组端粒酶活性增高 (56 %vs 42 % )。结论 端粒酶阳性组与端粒酶阴性组相比 ,在同等化疗强度的基础上 ,端粒酶阳性的AML患者完全缓解率 (CR)明显降低 (33 %vs 73 % ,P <0 0 5) ,表明端粒酶表达异常升高是造血细胞恶性程度高的标志之一 ,检测端粒酶的活性可协助白血病的诊断分型 ,并有助于白血病患者的疗效观察
Objective To observe and explore the telomerase activity in patients with acute myeloid leukemia (AML) with abnormal antigen expression and its prognosis. Methods Telomerase activity in bone marrow of 30 patients with AML was detected by telomeric repeat polymerase chain reaction (TRAP PCR). Results Twenty-five patients with AML had telomerase positive, with a positive rate of 50%. Ly + group (Lymphocytic leukemia cells, Lymphocytic leukemia cells) Ly Lymphocytes (Lymphocytes group) Lymphoid antigen expression was Ly-AML) telomerase positive rate is inconsistent. The telomerase activity of Ly + group was higher than that of Ly + group (56% vs 42%). Conclusion The telomerase positive AML patients showed a significantly lower CR (33% vs 73%, P <0.05) on the basis of the same chemotherapy intensity compared with the telomerase negative group ), Indicating abnormal expression of telomerase is one of the hallmarks of hematopoietic malignancy, the detection of telomerase activity can assist the diagnosis of leukemia typing, and contribute to the efficacy of leukemia patients observed
