患者,女,76岁。绝经25年,发现下腹部肿块10余天伴腹胀。妇科检查:子宫正常大小,子宫左上方可及约25cm×22cm×16cm大小肿块,质地不匀,大部分系囊性,部分实质性,活动尚可,表面有结节感。拟诊卵巢肿瘤行剖腹探查术,术中见少量淡黄色澄清腹水,肝脾表面光滑,肠系膜淋巴结及腹腔淋巴结均未及。宫体大小正常,右侧附件及左输卵管正常,左卵巢可见一肿块如上述。以囊性为主,部分区域结节状实质性突起,与周围组织无粘连。囊
