目的探讨植皮后封闭式负压引流术(VSD)治疗Ⅲ°烧伤的疗效。方法将54例患者随机分为单纯切痂植皮组和植皮后VSD组,各27例。观察及比较两组植皮成活率、感染率及治愈时间。结果切痂植皮组术中出血量为(55.3±1.3)ml/100 cm2,治愈时间为(17.6±1.2)d;植皮后VSD组术中出血量为(63.5±3.5)ml/100 cm2,治愈时间为(14.5±1.1)d。两组术中出血量和治愈时间差异均有统计学意义(<0.05)。两组皮片成活情况差异无统计学意义(>0.05)。两组仅有1例在切痂植皮组中出现局部感染,能有效控制。结论植皮后VSD治疗Ⅲ°烧伤皮片成活满意度佳,明显缩短了病程,减少住院天数,为患者早期康复打下基础。
Objective To investigate the curative effect of closed negative pressure drainage (VSD) for the treatment of Ⅲ ° burns after skin grafting. Methods 54 patients were randomly divided into simple excision excision skin graft group and VSD skin grafting group, 27 cases in each. The survival rate, infection rate and cure time of the two groups were observed and compared. Results The amount of hemorrhage was (55.3 ± 1.3) ml / 100 cm2 and the healing time was (17.6 ± 1.2) days in the eschar excision skin graft group, and 63.5 ± 3.5 ml / 100 cm2 in the VSD group after skin grafting The time was (14.5 ± 1.1) d. There was significant difference between the two groups in the amount of bleeding and the healing time (<0.05). There was no significant difference in survival between the two groups (> 0.05). Only one of the two groups had a localized infection in the eschar cuticle grafting group and was able to effectively control it. Conclusion The survival rate of VSD treated with skin grafting Ⅲ ° burn skin graft is good, which significantly shortens the duration of the disease and reduces the number of days of hospitalization, laying a foundation for the early rehabilitation of the patients.
