目的 探讨老年 2型糖尿病患者跟骨定量超声 (QUS)和腰椎骨密度 (BMD)的改变。 方法 采用QUS骨量分析系统检测 94例老年 2型糖尿病患者和 90例健康对照者的跟骨超声传导速度 (SOS)、超声振幅衰减 (BUA)及骨强度指数 (STI) ,用双能X线骨吸收测量仪 (DXA)测定前后位腰椎BMD ,并同时进行了骨生化指标的检测。 结果 2型糖尿病患者BMD(g/cm2 )男性L2 为 0 86 3,低于对照组的 0 931(P <0 0 5 ) ;女性L2 、L2~ 4 分别为 0 82 6和 0 841,均低于对照组 0 90 3和 0 90 5 (P <0 0 1) ;2型糖尿病患者跟骨BUA(dB/MHz)男、女分别为 6 4 6 3、5 1 0 5 ,均低于对照组 71 38和 6 1 76 (P<0 0 1) ,糖尿病组STI男、女分别为 83 12 %和 6 7 6 4% ,低于对照组 91 2 4%和 79 0 3% (P <0 0 5 )。糖尿病组女性SOS(m/s)为 1487 2 8低于对照组 15 0 2 34 (P <0 0 5 )。 结论 老年 2型糖尿病患者跟骨BUA、SOS和腰椎前后位BMD均较正常对照组降低 ,提示有骨量丢失
Objective To investigate the changes of quantitative calyx (QUS) and lumbar spine bone mineral density (BMD) in elderly type 2 diabetic patients. Methods The calcaneus ultrasonic conduction velocity (SOS), ultrasonic amplitude attenuation (BUA) and bone strength index (STI) were measured in 94 elderly patients with type 2 diabetes mellitus and 90 healthy controls by QUS mass spectrometry system. Bone resorption measuring instrument (DXA) before and after determination of lumbar spine BMD, and simultaneous detection of bone biochemical markers. Results In type 2 diabetic patients, L2 was 0 863 in BMD (g / cm2), lower than 0 931 in control group (P0 05); female L2 and L2 ~ 4 were 0 82 6 and 0 841 respectively In control group 0 903 and 0 905 (P <0.01), the calcaneal BUA (dB / MHz) in type 2 diabetic men and women were respectively 6 4 6 3,5 1 0 5, which were lower than those in control group 71 38 and 6 1 76 (P 0 01). The STI male and female in the diabetic group were 83 12% and 67 74% respectively, lower than 91 2 4% and 79 0 3% of the control group (P 0 01) 5). SOS (m / s) in diabetic group was 1487 2 8 lower than that in control group 15 0 2 34 (P <0 05). Conclusions The calcaneal BUA, SOS and anteroposterior BMD of lumbar vertebrae in elderly type 2 diabetic patients are lower than those in normal control group, suggesting a loss of bone mass
