目的:探讨射频消融联合无水酒精局部注射治疗术后复发肝细胞癌的临床效果。方法:将2011年8月—2015年1月诊治的术后复发肝细胞癌患者72例,根据随机数字表法分为治疗组与对照组各36例,对照组给予超声引导下经皮无水酒精注射治疗,治疗组给予射频消融联合无水酒精局部注射治疗,治疗观察周期为3个月。结果:所有患者都顺利完成治疗,治疗组与对照组的局部客观缓解率分别为72.2%、44.4%(P<0.05);治疗后,两组的外周血白细胞介素6(IL-6)和转化生长因子β(TGF-β1)都明显下降(P<0.05),治疗组的外周血IL-6和TGF-β1值明显低于对照组(P<0.05)。两组治疗前后的血清AST和ALT比较差异无统计学意义(P>0.05)。治疗组治疗期间的胸腔积液、静脉血栓形成、疼痛、发热等并发症发生情况明显少于对照组(P<0.05)。结论:射频消融联合无水酒精局部注射治疗术后复发肝细胞癌能提高机体的免疫状态,对于肝功能的影响比较小,减少并发的发生,提高局部客观缓解率,有很好的应用效果。
Objective: To investigate the clinical effect of radiofrequency catheter ablation combined with local injection of anhydrous alcohol on postoperative recurrence of hepatocellular carcinoma. Methods: Seventy-two patients with recurrent hepatocellular carcinoma who were diagnosed and treated from August 2011 to January 2015 were divided into treatment group (36 cases) and control group (36 cases) according to random number table. The control group was given ultrasound-guided percutaneous dehydration Alcohol injection therapy, the treatment group given radiofrequency ablation combined with local injection of anhydrous alcohol treatment, treatment observation period of 3 months. Results: The objective response rates of the two groups were 72.2% and 44.4%, respectively (P <0.05). After treatment, the levels of interleukin 6 (IL-6) and (P <0.05). The levels of IL-6 and TGF-β1 in the peripheral blood in the treatment group were significantly lower than those in the control group (P <0.05). There was no significant difference in serum AST and ALT between the two groups before and after treatment (P> 0.05). The incidence of complications such as pleural effusion, venous thrombosis, pain and fever during the treatment group was significantly less than that of the control group (P <0.05). Conclusion: Radiofrequency ablation combined with local injection of anhydrous alcohol in the treatment of postoperative recurrent hepatocellular carcinoma can improve the immune status of the body, and has less effect on the liver function, reduce the occurrence of complications and improve the local objective response rate, and has a good application effect.
