This article reports a case of intravenous infusion of streptokinase left main coronary recanalization, a large area of myocardial rescue and cardioversion cases of heart failure. Female, 79 years old, was admitted to hospital for cardiogenic shock. The examination showed extensive acute infarct on the anterior wall. Patients were intravenously infused with 500,000 units of streptokinase two hours after infarction, injected over 20 minutes, and anticoagulated with heparin. After 12 minutes, ECG showed signs of ST segment elevation and intracavitary conduction changes recanalization, Q wave did not occur. Fibrinolysis significantly increased after 2 hours
