目的 探讨伴有瞳孔闭锁或膜闭的 2 3例 (2 3眼 )并发性白内障在白内障手术中采取瞳孔塑形的临床疗效。方法 在 ECCE或 ECCE+IOL 中 ,剪除瞳孔区机化膜 ,从瞳孔缘环形剪除虹膜卷缩轮前 0 6 5～ 1mm的全层虹膜组织 ,在 1/ 10 0 0肾上腺素作用下使瞳孔散大至 5～ 6 mm ,术中截囊等步骤可在直视下操作。结果 术后视力 <0 .1者 2眼 ,≥ 0 .3者 2 1眼 ,最好矫正视力达 0 .9。瞳孔呈园形或近园形 ,直径 3～ 5 mm ,对光反应存在。术后随访 6～ 2 8月 ,无一例出现瞳孔后粘连和机化膜。结论 伴有瞳孔闭锁或膜闭的并发性白内障 ,在施行白内障摘除术时作瞳孔塑形术 ,增加了手术安全性 ,避免了术后并发症 ,疗效满意。
Objective To investigate the clinical efficacy of cataract surgery in cataract surgery with cataract in 23 patients (23 eyes) with cataract and membrane closure. Postoperative follow-up 6 ~ 28 months, no case of posterior pupil adhesion and mechanical membrane. Conclusions Concurrent cataract with pupil atresia or membranous obstruction is a pupil plastic surgery for cataract extraction, which increases the safety of operation and avoids postoperative complications. The curative effect is satisfactory.
