目的 探讨巨大垂体腺瘤显微外科手术治疗方法及经验。方法 对 5 6例巨大垂体腺瘤 ,根据肿瘤的生长方向及部位进行分型 ,根据分别采用经蝶、经额下、额下经蝶、扩大经蝶、扩大额下硬膜外、额下 翼点等 10种手术入路 ,并在显微镜下实施手术治疗。结果 5 6例巨大垂体腺瘤全切 2 9例、近全切 2 0例 ,大部切除 7例 ;无死亡。经平均 2年多的随访 ,证明手术治疗效果良好。结论 依据巨大垂体腺瘤的不同位置及生长方向选择适当的手术入路是提高全切率、降低死亡率及复发率的重要手段
Objective To investigate the microsurgical treatment and experience of giant pituitary adenomas. METHODS: A total of 56 giant pituitary adenomas were classified according to their growth direction and location. According to the results, they were treated with butterfly, subfrontal, suborbital, expanded transsphenoidal, subfrontal epidural, and frontal and inferior wings respectively. Point 10 surgical approaches and perform surgery under a microscope. RESULTS: Totally 29 cases of massive pituitary adenomas were treated in 20 cases, 20 cases were completely cut, and 7 cases were totally resected. No death occurred. After an average of more than 2 years of follow-up, it proved that the surgical treatment was effective. Conclusion Selecting the appropriate surgical approach according to different locations and growth directions of giant pituitary adenomas is an important means to increase total cut rate and reduce mortality and recurrence rate.
