目的 :总结纤维支气管镜对 19例危重病人的应用 ,评价其对危重病人气道管理的意义。方法 :在建立人工气道、高频机械通气条件下 ,急诊床旁纤维支气管镜检 ,吸引清除支气管内分泌物和肉芽组织 ,解除气道阻塞。结果 :9例肺部感染患者经纤支镜治疗 ,8例感染控制 ;6例肺不张患者经纤支镜治疗肺复张 ;3例气道损伤患者得以明确气道狭窄性质 ,并使气道梗阻有所减轻 ;1例气管肿瘤行气管支架植入。结论 :当危重病人大量呼吸道分泌物潴留致呼吸衰竭、肺不张时 ,应用纤支镜治疗是一种非常迅速有效的方法
OBJECTIVE: To summarize the application of fibrobronchoscopy in 19 critically ill patients and to evaluate its significance for airway management in critically ill patients. Methods: Under the conditions of artificial airway and high-frequency mechanical ventilation, emergency bedside fiberoptic bronchoscopy was performed to attract bronchial secretions and granulation tissue to relieve airway obstruction. Results: Nine patients with pulmonary infection were treated with bronchofiberscopy and eight with infection control. Six patients with atelectasis were treated with bronchofiberscopy to treat pulmonary recanalization. Three patients with airway injury were identified with airway stenosis and gas Road obstruction reduced; 1 case of tracheal tumor tracheal stent implantation. CONCLUSIONS: The use of fiberoptic bronchoscopy is a very rapid and effective method for the treatment of respiratory failure when a large number of critically ill patients have respiratory secretions
